Learning Objectives:

  • Discuss the benefits of implementing structured end-of-life communications for patients, in terms of social/emotional, financial and clinical outcomes.
  • Demonstrate an understanding of the five "conversation ready" principles identified by the Institute for Healthcare Improvement: Engage, steward, respect, exemplify and connect.
  • Examine and evaluate results of at least one pilot project to measure results and operationalize stewardship of records, thereby demonstrating respect for individual end-of-live care wishes.
